Description of Work:
The work includes widening I-95 from six lanes to ten lanes along this section, a length of approximately 1.8 miles. The work includes widening the existing Lake Worth Viaduct bridge, replacing the existing bridge over 6th Avenue South, and reconstructing the mainline ramps. The project will also include drainage improvements, sound barrier walls, retaining walls, signing and pavement marking, lighting, landscaping, irrigation, and communication elements.

Special Considerations:
Time plus Money (A+B) Bidding:  This is a special type of bidding process where the Contractor bids the cost of the job (A, or the "Standard Bid") plus the number of days in which the project will be completed (B, or the "Time Bid").   The Time Bid is then multiplied by a daily dollar value and the Standard Bid and the Time Bid are added together to get the Total Bid.  The project will then be awarded to the bidder with the lowest Total Bid.  The actual contract duration is therefore set by the Contractor during bidding.  The purpose of this type of bidding is to encourage the Contractor to complete the project in the shortest possible time.

No Excuse Bonus:
The No Excuse Bonus concept is to provide the contractor with substantial incentive to complete a project within a specified time frame regardless of any problems or unforeseen conditions that may arise.

Bonus #1: $2,000,000 Complete all contract work and obtain acceptance in 1000 days.
Bonus #2: $10,000 to complete 10th Avenue SB On-Ramp (Ramp 2-A1) in 24 consecutive days from initial closure.
Bonus #3: $60,000 to complete 6th Avenue SB On-Ramp (Ramp 1-A1) in 148 consecutive days from initial closure.
Bonus #4: $60,000 to complete 6th Avenue SB Off-Ramp (Ramp 1-D1) in 190 consecutive days from initial closure.
Bonus #5: $60,000 to complete 6th Avenue NB Off-Ramp (Ramp 1-B1) in 119 consecutive days from initial closure.
Bonus #6: $60,000 to complete 6th Avenue NB On-Ramp (Ramp 1-C1) in 167 consecutive days from initial closure.

Incentive/Disincentive:
The Incentive/Disincentive concept is used to expedite the completion of certain aspects of the project.  The number of I/D days will not be changed for any reason, including, but not limited to, work added, work deleted, bad weather, unforeseen conditions, etc.

Project: I/D daily amount of $10,000. Maximum I/D: $300,000; 1000 Calendar Days from commencement of Contract Time.
Ramp 2-A1 (10th Avenue N SB On-Ramp) I/D daily amount of $3,000. Max I/D: $15,000; Max closure time 24 days.
Ramp 1-A1 (6th Avenue S SB On-Ramp) I/D daily amount of $3,000. Max I/D: $60,000; Max closure time 148 days.
Ramp 1-D1 (6th Avenue S SB Off-Ramp) I/D daily amount of $3,000. Max I/D: $90,000; Max closure time 190 days.
Ramp 1-B1 (6th Avenue S NB Off-Ramp) I/D daily amount of $3,000. Max I/D: $60,000; Max closure time 119 days.
Ramp 1-C1 (6th Avenue S NB On-Ramp) I/D daily amount of $3,000. Max I/D: $75,000; Max closure time 167 days.

Other Special Provisions:
Article 7-11.1.1: Routine Maintenance: Sweep the I-95 roadway and bridge surfaces every 14 days, and the side streets every 7 days.  Provide daily clean up of litter, debris and dirt from local streets and properties. Failure to clean up in 12 hours, FDOT has right to clean up and back-charge the contractor.

Article 7-11.3.1: Hauling and staging: Prohibits the contractor from parking, staging and/or hauling material and equipment using local surface streets except for the construction of the Lake Worth High School parking lots.

Article 8-3.3: Prosecution of Work- Mandatory Start: Contract Time will start on July 6, 2004.  The Contractor will use the time from Notice to Proceed to July 6, 2004 to procure and fabricate materials, for the submittal and review of shop drawings, for the submittal and review of the schedule, to participate in a Constructability/Value Engineering Workshop, and to adjust work forces, equipment and schedules to proceed in a manner to minimize disruption to the public.

Article 8-3.7: Disputes Review Board: The Board will provide special expertise to assist in and facilitate the timely and equitable resolution of disputes, claims, and controversies between the Department and the Contractor in an effort to avoid construction delay and future claims.

Article 8-4.1.1: Sequence of Operations:

Lake Worth High School Parking Improvements:  The Lake Worth High School Parking Improvements will begin July 6th, 2004 and shall be completed in their entirety prior to September 15th, 2004.  No other work under the contract shall begin until the Lake Worth High School Parking Improvements are completed and accepted by the Engineer.  The Contractor shall not use the area of the Lake Worth High School Parking Improvements for staging, parking or any other purpose after completion and partial acceptance of that work.

Ground-Mounted Sound Barrier Walls:  The Ground-Mounted Sound Barrier Walls included in this contract shall be constructed and completed in Phase 1 of this contract.  No work designated in the Traffic Control Plans as part of Phase 2 shall begin until the Ground-Mounted Sound Barrier Walls have been completed.

Traffic Control Plan Phasing:   The Contractor shall focus his efforts and schedule his work forces to expedite completion of Ramp 2A1 within the first 25 days of Phase 2, completion of Ramp 1A1 within the first 155 days of Phase 2, and completion of Ramp 1D1 within the first 200 days of Phase 2.  The Contractor shall focus his efforts and schedule his work forces to expedite completion of Ramp 1B1 within the first 125 days of Phase 3.  The Contractor shall not begin work on Ramp 1C1 until completion of the ramp becomes critical in the Contractor’s schedule.  This phasing is being done in order to minimize the duration of ramp closures. 

Staged HOV Lane Opening and Coordination: The Department desires to open new HOV lanes in a manner that ensures segments of suitable length are opened at the earliest practical opportunity.  HOV lanes from Lantana Road to South of SR 80 are expected to open in June of 2007.

Article 8.4-10: Protection of Adjacent Properties: Any equipment used by the Contractor for this project shall not exceed vibration levels of 0.20 inch per second at any point within 100 feet of the right of way. The Contractor shall employ a qualified Specialty Engineer to monitor and record vibration levels produced by vibratory construction equipment used on this project.  The Contractor shall perform a Pre-Construction survey with the use of video and still photography on each property, located within 400 feet from the Right of Way, identifying the existing structural conditions, of the property or any other improvement of such property which may be affected by vibration. In addition, a limit of 1000 feet will be used to monitor vibration levels around Lake Worth High School and the City of Lake Worth Water and Electric facilities.

Article 8-4.11: Noise Restrictions: Equivalent sound levels shall not exceed 50dba Monday - Friday from 8 p.m. to 8 a.m., all day Sunday and during legal holidays.
